The basics

What are exosomes?

Exosomes are a type of extracellular vesicle: membrane-bound particles about 100 nanometers across that cells release to communicate with one another (PMID 32029601). Each one carries a cargo of proteins, growth factors, lipids and RNA that reflects the cell it came from.

When another cell takes up an exosome, that cargo can change how the cell behaves. Exosomes take part in inflammation, immune signaling and tissue repair, which is why regenerative medicine is now putting this signaling system to clinical use.

Exosomes vs stem cells

Exosomes vs stem cells: what is the difference?

Stem cells are living cells that can divide and develop into other cell types. Exosomes are the messengers those cells release. They carry the cell’s signals, including growth factors and RNA, without the cell itself, which is why researchers describe exosome therapy as a cell-free form of regenerative medicine (PMID 28294454).

Sourcing

Where do our exosomes come from?

Exosome products differ in where they come from, how they are made, and how pure and concentrated they are. Clean, well-tested exosomes are hard to find, so we are careful about ours. They are derived from donated umbilical cord tissue, and our source:

  • Uses only healthy U.S. donors after planned C-section births
  • Screens every donor’s health history, with final approval by a licensed physician
  • Tests donor blood in a CLIA-certified lab for HIV, hepatitis B and C, syphilis, HTLV, West Nile virus, Zika and CMV
  • Keeps each vial to a single donor, with a lot number and donor ID that can be traced
  • Processes in an FDA-registered lab that follows cGMP (Current Good Manufacturing Practice) guidelines
  • Holds every batch in quarantine for two weeks until bacterial testing is complete, and tests for endotoxin and bioburden before release
Overseen by our doctors. Every exosome treatment at Nature & Science Medicine is given here in our Encinitas clinic, under a doctor’s care.

The research

What does the research show for exosomes?

The largest review to date is a 2026 meta-analysis of 39 human studies of exosome therapy in aesthetic medicine, published in Aesthetic Surgery Journal (PMID 41800726). Here is what it found.

  • 20.2%
    Facial wrinkles

Average reduction in wrinkles across the clinical studies.

  • 23.4%
    Skin quality

Improvements of 14.7% to 23.4% across measures of skin quality, including pigmentation and elasticity.

  • 23.6%
    Hair density

    Average improvement in hair density.

  • 18.0%
    Hair thickness

    Average improvement in hair thickness.

  • 32.5%
    Acne scars

In a double-blind, randomized split-face trial, the side treated with exosomes after fractional laser improved 32.5%, versus 19.9% on the control side, with milder redness and shorter downtime (Acta Dermato-Venereologica, 2020, PMID 33073298).

Beyond skin and hair

Researchers are also studying exosomes for tissue repair and wound healing, for calming inflammation, as carriers for targeted drug delivery, and as blood biomarkers for detecting and monitoring disease (PMID 32029601).

Exosome therapy questions

What is exosome therapy?

Exosome therapy uses exosomes, tiny cell-free messengers released by stem cells, to support the body's own repair signals. At Nature & Science Medicine in Encinitas, our doctors use carefully sourced exosomes in IV therapy and microneedling facials, and you plan your treatment one-on-one with Dr. Erin Biller, ND.

How are exosomes different from stem cells?

Stem cells are living cells that can divide and develop into other cell types. Exosomes are the messengers those cells release, carrying proteins, growth factors and RNA without the cell itself. Researchers describe exosome therapy as a cell-free form of regenerative medicine (PMID 28294454).

Where do your exosomes come from?

Our exosomes are derived from donated umbilical cord tissue from healthy U.S. donors after planned C-section births. Donors are screened and their blood is tested in a CLIA-certified lab, each vial comes from a single traceable donor, and every batch is quarantined and tested before release.

What is exosome therapy used for?

Exosomes are used to support skin rejuvenation, collagen production, scar remodeling and hair density, and they are being studied for tissue repair and wound healing. At our Encinitas clinic we offer exosome IV therapy and exosome microneedling facials.

What does the research show for skin and hair?

A 2026 meta-analysis of 39 human studies found an average 20.2% reduction in facial wrinkles, a 23.6% improvement in hair density and an 18.0% improvement in hair thickness after exosome treatments (Aesthetic Surgery Journal, PMID 41800726).

What is an exosome microneedling facial?

Microneedling creates tiny channels in the skin, and exosomes are applied during the treatment to support collagen and skin renewal. In a randomized split-face trial, skin treated with exosomes after fractional laser showed greater acne scar improvement and shorter downtime than the control side (PMID 33073298).

Do you offer stem cell therapy?

Yes. Dr. Erin Biller, ND offers both stem cell and exosome therapy at our Encinitas clinic. Both start with the same 30-minute consultation, where she reviews your health history and goals and explains which option fits you.

How much does exosome therapy cost?

The 30-minute exosome and stem cell consultation with Dr. Erin Biller, ND is $220. Your treatment plan and its price are set at that visit, based on the treatment you choose and how many sessions you need.
