Our Policies


Scheduling an Appointment

The best way to schedule an appointment with us is through our online booking page or by calling our office. We try our best to accommodate you when scheduling but please keep in mind, that each practitioner has specific availability. The further you schedule in advance, the higher the likelihood we will have your desired time slot available.


Before Your Appointment

Please fill-out the paperwork and set up your CharmEHR Patient Portal login info before your appointment, it takes about 30-45 minutes to complete all the paperwork. Intake paperwork and consent forms are sent electronically with your appointment confirmation and require you to log into our CharmEHR Patient Portal to access them. This helps us really delve into your case and understand you better before we start the collaborative healing process at Nature and Science Medicine.
Please fill out Intake Paperwork and Consent Forms to the best of your ability and at least 48 hours before your initial visit.


First Appointment

The first appointment is approximately 60 minutes. This time is to really get to know you and establish a therapeutic relationship. Your practitioner wants to understand your concerns and history to determine the root cause(s) and help with your imbalances. A thorough timeline and a physical exam is performed at this appointment.
Lab work is often ordered during the initial visit. Your practitioner will order testing based on your concerns. This may include: routine blood work, specialized testing, stool panels, micronutrient and food allergy testing, urine or salivary testing, etc.​


First Follow Up Appointment

This visit is comprised of lab results review. Your practitioner will go over your results in detail and answer any questions you may have. You will be given a thorough treatment plan that addresses your concerns. Follow up appointments are traditionally done In-Office but may also be done as a Phone or Video Appointment.


Additional Follow Up Appointments

Additional follow ups will vary for each patient. Appointments are usually spaced 4-6 weeks apart at the beginning and will be less often when on a maintenance plan. Follow up appointments are traditionally done In-Office but may also be done as a Phone Appointment.​


Cancellation Policy

Appointments can be rescheduled or canceled anytime up until 24 hours prior to your scheduled appointment time.

An appointment for a consultation with the doctor that is canceled or rescheduled with less than 24 hours notice will be charged the full appointment fee. However, if the appointment is rebooked within the next 14 days, the already paid appointment fee can be applied. 

Missed appointments with the doctor with no prior cancellation or rescheduling are charged the full price of the appointment.

A cancellation fee is charged out of respect for the time of each of our doctors and to ensure that appointment time slots are available to those who can attend.

If multiple appointments were scheduled back-to-back, appointment fees may be charged for each missed appointment.

For Ozone Therapy:

A missed appointment for ozone therapy fee is 50% of the price paid for the patient’s last ozone treatment.  If no prior ozone treatment has been completed, a flat fee of $200 will be collected.  If a package was purchased for ozone, the fee collected will be 50% off the package price for the visit.


Payment

Payment is due at the time of the service, and in some instances, may be due at the time of scheduling.  If the latter is the case, I will be notified by staff.  Nature and Science Medicine accepts: Cash, Credit, HSA/Flex Spending Cards, and Checks as well as PayPal and Apple Pay.


Prescriptions & Refills

Most medications require prescription monitoring. With every prescription, we dispense the maximum allowed number of refills before you need a follow up visit. Check your pill bottles for "number of refills remaining" and schedule a visit during the final month's refill. This allows you to get the careful monitoring you deserve to use prescriptions safely. Please make sure you mention in any visit with us if you are close to running out of anything so we can address it together in person.

Prescriptions which have run out of refills will NOT be refilled until proper follow up has been completed (usually this involves blood work and a follow-up appointment). For your safety and proper prescription management, there will be no exceptions to this policy.

Laboratory Testing

For laboratory testing, please understand what your health insurance policy will and will not cover. Up-front cash pay is often the best option for individuals whose insurance often does not cover eclectic laboratory testing, as labs offer discounted prepaid rates.


Patient Portal & Messaging

The best way to contact your doctor is through the CharmEHR Patient Portal. An appointment is required to fill out forms, order labs, give medical advice, prescribe/refill medications, recommend supplements, and perform other services.

We welcome informative messages via the patient portal. However any request listed above requires a scheduled appointment, as we can not give medical advice or provide medical services outside of appointments.
Phone calls with us must be scheduled as an appointment. Phone appointments cost $99 per 15 minute increment.

Messages on the phone and Charm EHR Patient Portal will be returned within 48 hours Monday-Thursday and 72 hours Friday-Sunday.
